    Monarch Primary Care treats patients 18 and older for a variety of typical health conditions, including:

    • Routine checkups
    • Immunizations and screening tests
    • Physicals
    • Diagnosis and treatment of illness
    • Health-risk assessments
    • Acute, chronic and preventative medical care

    We are open Monday through Friday, 9 a.m. to 5 p.m.. We accept Medicaid, Anthem or self-pay patients.


    Walk-ins are seen based on availability, but the best way to ensure your health concerns are addressed is to make an appointment.
